By law, Medicare-eligible patients are required to enter into a private contract with Lisa Bailer and care would then be provided on a fee-for-service basis. This fee is not reimbursable by Medicare. By accepting the treatment contract with Lisa Bailer. you agree that you shall not submit a claim for payment under Medicare for services rendered at our office.
